Bile and Cholesterol Stones Together: A Rare Case of Cholelithiasis

Introduction

Gallstones are common, but it is unusual to find both bile pigment stones and cholesterol stones in the same patient. Recently, he treated a patient with this rare combination, shedding light on the complexities of gallstone formation.

Case Details

A patient came in with recurring right upper abdominal pain, nausea, and vomiting, mostly after fatty meals. They had no significant medical history but followed a sedentary lifestyle with irregular eating habits.

Diagnosis

To uncover the cause of the symptoms, we performed:

  • Ultrasound: This revealed multiple gallstones without signs of inflammation.
  • Liver Function Tests: Normal results ruled out bile duct obstruction.
  • CT Scan: Confirmed the stones and excluded complications.

Surgical Findings

During laparoscopic gallbladder removal, we discovered two types of stones:

  1. Bile Pigment Stones: Small and dark, linked to bilirubin buildup.
  2. Cholesterol Stones: Larger and yellow, caused by excess cholesterol in bile.

This combination suggested a mix of dietary issues and bile stagnation as the underlying causes.

Treatment and Outcome

We performed a laparoscopic cholecystectomy, which went smoothly. The patient recovered well and left the hospital the next day with advice on lifestyle changes to prevent future issues.

Conclusion

This case shows how multiple factors can lead to gallstone formation. Early diagnosis, combined with minimally invasive surgery, ensures effective treatment and quick recovery.
